Forms for students at All Suburban Driving School
Proof of Eligibility

Blue Form	Waiver for students who have eight passing grades in the past two semesters.
Pink Form	Waiver for students who have not received eight passing grades in the past two semesters.
Yellow Form	Waiver for Home Schooled Students.
G.E.D. Requirements	Requirements for students enrolled in a G.E.D. Program

Forms for students that are enrolled in the BTW only program
Most students at All Suburban Driving School take the complete course which includes the classroom phase & the Behind the Wheel ( BTW ) lessons. The forms listed below are available for the students that take the BTW ONLY lessons with All Suburban Driving School.
These forms must be completed for students who have taken the classroom phase at their high school & are taking the BTW ONLY portion at All Suburban Driving School. We will also need a photocopy of your driving permit.
